Community Health Assessment survey

Madison Health Dept., in conjunction with the Morris Regional Health Partnership & TCNJ, is excited to announce the launch of a Community Health Assessment survey. The survey is a crucial component of a larger effort to gather data from various sources.

Hello Morris County Residents!

This survey is being conducted by The College of New Jersey who is working with Morris County Health Departments. We are hoping to get your opinions about things in your community that affect health and ideas for improving health. Please take a few moments of your time to complete this survey. Feel free to skip any questions that you do not want to answer. We appreciate your help in advancing the well-being of our community. Thank you!

Answering this survey is optional. We are recruiting participants who are age 18 or older. If you are not yet age 18 or older, please close this window and do not provide any further information.

The survey is anonymous. That is, we are not asking for information that would allow anyone to find out who you are or to link your answers back to you. The survey will take about 20 minutes or less to complete.

At the end of this survey, you will have an option to tell us more about your concerns or tell us a story in a small group Chat or an individual interview. To be in the Chat or Individual Interview, we will need you to please provide your email or phone number. That information will be stored in a separate database and your answers there cannot be linked to your answers in this survey. We will use the contact information to reach out to you for a small group chat or an individual interview to hear more from you about the community health concerns.
